We provide home-based quality care in the comfort of your home, work, or where ever you are. Chronic or acute illnesses, post-hospitalizations, barriers related to aging, work-related scheduling issues, and financial needs can affect patients' access to healthcare.

At On-Site Health, we understand that there are a number of reasons that may be keeping you from seeking healthcare from a Provider on a regular basis, so we are promoting wellness and healthier lifestyles by specializing in house calls for primary and acute medical care.  


OUR SERVICE AREAS INCLUDE: 

On-Site Health provides services to a 50-mile radius from the office address.  Our service area includes the following counties: Warren, Rankin, Hinds, Madison, Yazoo, Holmes, Scott, Simpson, and Leake Counties

Patient Centered Care

The Institute of Medicine defines patient-centered care as “Providing care that is respectful of, and responsive to, individual patient preferences, needs and values, and ensuring that patient values guide all clinical decisions.” By aligning our mission and values with this approach, we provide care that is collaborative, coordinated, and accessible. We aim to be in the right place, at the time with high-quality care.

Coordination of Care

Chronic illness requires a team of skilled health care providers to work together. We will coordinate between specialists, pharmacists, DME suppliers, and more to limit the trips into the clinic and provide visits and care in the home to decrease exposure and distress to the patient.

Assisted Living Facilities

Residents in assisted living facilities often have little access to transportation and rely on family members' schedules to see a healthcare provider. We can coordinate and meet a broad range of medical needs in collaboration with the facility staff to treat one or multiple residents.

Transitional Care

We want to help you avoid re-hospitalization. We can coordinate with your hospital healthcare providers and continue care as you transition from the hospital setting to home.  In the home, we can help identify condition and care needs inside your home environment as they arise throughout the course and healing of your acute or chronic illness.

Medication Management

We will individualize care plans and schedules to help patients manage their medications so they take them on time and every time. In order to avoid any incorrect medication administration, we have the unique ability to evaluate dosage and directions on all home medications in their original bottles with prescribed labels. In the event that a patient cannot afford medications, we will connect them to community and pharmaceutical resources to help with the costs or transition to another effective prescription within their price range.

Telehealth Visits

Telehealth increased health care value and affordability. By providing this service, we can save our patients even more time and money. We can address medication issues, urgent care issues, hormone replacement therapy, and much more.

Minor Urgent Care

Life is unpredictable. If you are having an emergency or experiencing a serious or life-threatening illness, please go to your local ER or call 911. If the condition is a minor illness or injury, then take advantage of the our convenient and affordable options and let us come to you.

DOT Physicals

A Department of Transportation (DOT) physical is a health examination mandated by the Federal Motor Carrier Safety Administration (FMCSA) for commercial motor vehicle (CMV) drivers. A DOT physical helps determine if a driver is physically, mentally, and emotionally fit to operate a CMV. We understand both the value of this examination and your time. We can offer employer contracted on-site physicals or individual appointments while on the road.
